Norfolk County Council and Great Yarmouth and Waveney CCG are working to improve the way we broker care and support discharges of patients from hospital to either their home or a care home. We are developing this scheme at the James Paget Hospital, where we want to provide patients with all the information they need to make choices about their care, from the facilities and services different providers offer to their prices and availability. We want to work much more closely with providers to be able to present patients with their options, to identify any issues around the efficient discharge of patients to a care package and ensure that this scheme resolves them in ways that helps patients make their journey out of the hospital and providers of care to support their clients.
This scheme will offer a number of benefits to providers. We want to work with care home and home support providers to improve communications, information sharing and the processes around a patient’s transfer to a care home or return to their own home to increase your level of confidence when accepting new patients, especially when they require complex care and support. We also want to deliver the information you share with us about your organisation directly to patients and their families to empower them to make choices about care and support prior to placement.
We will be hosting a market event at the Kings Centre, Great Yarmouth, on Tuesday 3 October to introduce the scheme, answer your questions and discuss how we can develop the scheme together. Refreshments will be available from 9:30, with the meeting commencing at 10:00 and ending by 12:30.
